Who is eligible?

The CWCS supports patients who are living with diabetes and have a chronic wound, and who meet the following criteria:

• Are 65 years or older, or are a First Nations person aged 50 years or older
• Are not currently receiving wound care support through another government-funded scheme

How does the CWCS work?

An eligible health professional will:

• Assess the patient’s wound

• Order the appropriate wound consumables through an online government portal

• Have the products delivered either directly to the patient’s home or to their clinic at no cost to the patient.
